Amazon Cargo Plane's Fatal Runway Overrun Raises Concerns About Pilot Communication and Aircraft Deceleration
A preliminary investigation into a fatal runway overrun at Miami International Airport has revealed that one of the pilots repeatedly expressed concern about the Boeing 767's speed during the final stages of the approach and landing.
The aircraft, a Boeing 767-300 freighter operated by 21 Air for Amazon Prime Air, arrived at Miami International Airport from San Juan, Puerto Rico on September 6, 2026. The crew received several warning cues before the aircraft overran the runway, including automated alerts related to descent rate and terrain proximity.
The pilot's speed concerns continued for more than a minute before the cockpit recording ended, but there was no consistent verbal response from the other pilot. Investigators will examine whether the approach met operational criteria and what opportunities were available to discontinue the landing.
