Merck's Cancer Combination Strategy Faces Manufacturing Hurdles

Pharmaceutical company Merck is developing new cancer treatments by pairing antibody-drug conjugates (ADCs) with its popular immunotherapy drug Keytruda. The goal is to improve treatment outcomes for breast and gynecologic cancers while preparing for the patent cliff of Keytruda.

The strategy focuses on patient tolerability alongside efficacy, aiming to improve outcomes without adding significant toxicity. Merck's investigational ADC sacituzumab tirumotecan recently showed improved overall and progression-free survival in a phase 3 trial for previously treated endometrial cancer.

Merck is also testing ADCs alone and with Keytruda across several women's cancers, seeking to replace chemotherapy in some regimens. However, personalized cancer vaccines, such as those developed by Merck and Moderna, face manufacturing hurdles and commercial challenges due to the need for sequencing, target selection, and individualized batch production.

Prediction markets have raised new risks for pharmaceutical companies, allowing people to wager on clinical trial outcomes and FDA decisions.
