Tennessee's Health Hubs Expand to Reach Over 200,000 People
Health hubs in Tennessee are providing preventive care and health coaching to underserved communities through partnerships between UnitedHealth Group and UT Health Sciences. The program has already shown success, with over 4,000 people served across five health hubs, resulting in a 12% decrease in average blood glucose levels and more than half of participants lowering their blood pressure.
The $4 million commitment will expand the program to 13 locations across Tennessee by the end of 2027, aiming to reach over 200,000 people. This investment will also support UT Health Sciences College of Nursing and UT Health Sciences College of Medicine to advance career pathways in rural areas, helping bring more healthcare professionals to parts of the state where care is sparse.
The key to this program's success lies in building trust through local health coaches who provide personalized support and education. As one participant noted, 'I could share things with my health coach I wouldn't share with my doctor.'
This model has been recognized by health systems across the country, with UnitedHealth Group committing $20 million from the United Health Foundation to start scaling local health solutions like this across the nation.
