Amazon Prime Air 767 Crash: Pilots Received Seven Warnings Before Fatal Go-Around

A Boeing 767-300F operated by Amazon Prime Air crashed at Miami International Airport (MIA) on Sunday, resulting in five fatalities. The National Transportation Safety Board (NTSB) has released a summary of cockpit voice recorder data from the incident.

The recordings show that the crew received seven automated warnings in the final moments of approach, including two 'sink rate' and five 'too low terrain' alerts. One pilot repeatedly warned the other about the aircraft's excessive speed more than a minute before touchdown.

The NTSB states that there was not a consistent verbal response to the warnings about airspeed. The crew then attempted a go-around approximately 16 seconds after touchdown, but it was aborted just four seconds later, leaving the 767 with only seconds of runway remaining before it left the paved surface.

Investigation has revealed that the aircraft was late in adopting its proper landing configuration, with Flaps set to five degrees at two minutes and 53 seconds. The jet dropped 400 feet in just four seconds before touching down close to the halfway point of Runway 30.
