UnitedHealth Soars on Q2 Earnings Beat: Medical Benefit Ratio Hits Two-Year Low
UnitedHealth Group Inc (NYSE: UNH) delivered strong Q2 2026 results, beating analyst estimates by a significant margin. The company reported adjusted earnings per share (EPS) of $6.38, a 30% upside from the expected $4.91. This marks the largest quarterly EPS beat in some time for UnitedHealth.
The medical benefit ratio, a key metric that impacts bottom-line earnings, improved to 86.7% in Q2 2026 from 89.4% in Q2 2025. This decline represents a 270 basis point improvement and translates to roughly $3 billion in additional operating income annually. The company's deliberate actions to reduce medical costs, including changes to health insurance benefits and product pricing strategies, contributed to this improvement.
UnitedHealth also saw a significant rebound in Optum operating income, which rose 29% to approximately $4 billion. Additionally, the company generated $11.1 billion in operating cash flow, equivalent to 1.9 times net income. These metrics suggest that cost reductions are not one-off items.
The company lifted its full-year 2026 guidance for adjusted EPS to $19.50 to $20.00 and raised its revenue forecast to over $439 billion. Piper Sandler boosted its stock rating and price estimate for UnitedHealth, moving it from $420 to $475 in response to the Q2 results.
